Development update
New All Events dashboard
Previously, teams would spend time navigating between the legacy Cases list, a separate Search page, and the Summary pages. For an improved workflow, we have combined the Cases list and the Search page into a single, easy-to-use All Events dashboard. This update streamlines your workflow by combining event review and management into one page.
What’s new:
Unified workspace: Search by ID, facility, or keyword directly alongside your work queues. Anonymous submissions and linked events now live in the same secure view (anonymized based on your access level) rather than being siloed.
Quick summary and views: Choose between a structured list or a visual card layout. Use the new Quick Summary tool to review vital event details without leaving your queue or opening a new tab.
Visual event badges: New visual badges immediately highlight overdue actions, learning points, and failed integrations, ensuring regulatory compliance and successful data transmission.
Aging and workflow visibility: Next to your Workflow Stage, you can now see exactly how many days an event has been present. The Days Open filter (ex. Aging 30+ Days) allows for long-running work tracking.
Smarter assignment and bulk actions: You can assign events to a team, user, or both simultaneously (selecting a Team filters the User list). We have also optimized our bulk-action tools, allowing you to perform multi-event assignments, workflow stage updates, and event closures all at once.
Global navigation refresh
To support the new All Events dashboard, we have refreshed and improved the platform's sidebar and header navigation. We have simplified the terminology and grouped modules to match how you work.
What’s new:
Grouping tabs: Navigation is now divided into Core Events, Modules, and Administration tabs. Tabs then expand (ex. clicking Event Management displays Dashboard, All Events, CQI Meetings, and Data Integrations options). Smooth flyout menus (ex. hovering over Insights & Analytics) display as you navigate.
Submit from anywhere option: You can now Submit an Event directly from the main sidebar, under Core Events.
Focused "Submit Only" view: Users with Submit Only access will see only Submit an Event and their Dashboard.
Clearer terminology: For an improved user experience, module names have been refined. Procedures is now SOPs, and the legacy Data Warehouse is now Benchmarking.
Repositioned controls: Changing your active facility (under YOU ARE LOGGED INTO) and submitting Product Feedback now display at the bottom of the sidebar.
Transfer ownership, automated workflows and actions
When working with multi-site events and linked facilities, we have improved the workflow of handing off primary control. We have also set up automated rules to ensure the right facilities receive emails and follow-up Actions in real-time.
What’s new:
Transfer ownership: When moving the primary control of an event to another linked facility, the new facility becomes the Primary Event Owner, while the previous owner stays linked with full access. The Original Submitter remains permanently visible for auditing, and the new owner is notified based on your configured notifications rules.
Correcting mistakes: The Transfer Ownership option is for intentional workflow handoffs. If an event was submitted under the incorrect location by mistake, continue to use the Change Facility option.
Automated workflows: We have redesigned Form Setting Notifications into clearer, highly targeted Automated Workflows. You can now set up rules to automatically send emails or create Actions at specific journey points: When an event is initially submitted, when a new facility is linked, or when ownership is transferred.
Automated actions: Workflows can now automatically create and assign follow-up Actions. You can assign these by staff position or to the original report submitter. To act as a safety net for vacant roles, you can also configure alerts to trigger if an automated action remains unassigned.
Clearer access levels: On the Event Management page, it is now clear who can take action. Owners have primary control to manage and close the event, while linked participants are granted specific roles (Full Access, Summary Page Tools, or Discussion Only) allowing them to contribute safely at their designated level without being blocked by ownership restrictions.
Form field settings performance redesign
Form configuration is now faster and safer. Administrators can manage form sections and questions individually, keeping field rules consistent organization-wide without the risk of misconfiguration.
What’s new
Enhanced configuration: Organizations with existing permissions to add custom questions can now configure standard questions one at a time (Mandatory, Optional, or Disabled). Hidden options are reviewable via the Show more options toggle highlighted in yellow.
Note: We are actively working to expand the coverage of this tool. You may notice that certain sections or questions are currently hidden from this configuration view on specific forms.
System guardrails: Required Starting Questions and System Integrated Fields (ex. LFPSE, CMIRPS, CAMER) are locked by the system to prevent accidental disabling that could affect external reporting.
Reporting impact warning: Fields connected to Insights and Benchmarking are not currently flagged in this update. If you disable a question that feeds these reports, the associated charts will stop working. Please review carefully before disabling fields.
Patient tool bypass: A new Allow users to bypass patient details based on relevance setting adds a simple Yes or No prompt to forms, allowing users to skip patient details if a patient was not involved.
Risk matrix helpers: To improve scoring consistency, hover-helpers now guide users on Potential Harm and Likelihood of Recurrence. The system also actively prevents the creation of duplicate Failure Modes.
CQI meeting tools and Insights Pro improvements
We have made CQI meetings easier to run and share securely. We have also improved the usability of date pickets and system messages across the platform.
What’s new:
Governed PDF downloads and audit logs: A new Download CQI Meeting PDF permission controls who can export official meeting records. To ensure accountability and security, the system now stores audit logs in the database, tracking who downloads these PDFs. We have also resolved text-alignment issues, ensuring long discussion notes paginate cleanly in the final export.
Clearer meeting setup: Required fields now display an asterisk (*). Near Miss has been renamed to Good Catch within Insights and PDFs to match the rest of the application.
Insights Pro multi-export: Users can now select and run multiple report types (ex. Form Data, Root Cause, Actions) in a single export action, rather than running them one by one.
Easier date selection and notifications: Date selections now feature month/year dropdowns for faster historical navigation. Range pickers clearly display dual From and To calendars. Success and Error messages can now be dismissed manually.
6. Admin access, Settings, and profile updates

What’s new:
Bulk user-to-facility assignment: Clients can now manage facility access at scale by uploading a standardized CSV via Settings > Users.
SSO troubleshooting: Corporate sign-in failures now provide specific, actionable error messages (ex. distinguishing between No matching Login ID and SSO not enabled on profile).
My Profile enhancements: Custom profile photos have been replaced with auto-generated initials avatars. Social login users (Google/Microsoft) can now set a traditional password via a unified setup flow.
Inactive account handling: Attempting a password reset on an inactive account no longer emails the user or alerts administrators. The system now provides an on-screen message to contact an admin for activation.
Temporary password and SOPs: Temporary Password is now a standard core administrator action (which no longer requires a separate module toggle). SOP Sign-Off Assessments have been moved under SOP Management.
Regional and regulatory updates
🇨🇦 Canada: Pharmacy Medication Incident and Good Catch
To improve the reporting workflow, we have simplified the Canadian reporting forms by making non-ISMP NIDR fields optional where possible.
What’s new:
Streamlined Incident and Good Catch: Incident and Good Catch forms now feature fewer required fields. By making non-regulatory questions optional, users can complete "Required-First" reporting much faster.
Incident form update: The Pharmacy Medication Incident form features a new Dose Status / Disruption path, making it much easier to categorize "Medication incorrectly taken" or "Medication missed" incidents.
"Show Required Only" defaults: Administrators can now set the default behavior for forms (e.g., defaulting the Good Catch form to hide optional fields for speed, while leaving the Incident form fully expanded for comprehensive clinical detail).
🇬🇧 UK: LFPSE v6 compliance
Pharmapod is now fully compliant with the NHS England LFPSE v6 API standard.
What’s new:
Medication Incident form update: We have introduced comprehensive updates to the Medication Incident form to meet v6 standards. This includes new multi-selects for stages of care delivery, updated risk/harm wording (including guidance for incidents affecting 10+ patients), refreshed device and IT system detail fields, a major responsible specialty list refresh, and new PSIRF (Patient Safety Incident Response Framework) governance questions.
Good Care (LFPSE) form enhancements: The Good Care form now features expanded Reporter Role options (including a new, auto-selectable Pharmacist role to save time). It also includes a new mandatory field capturing the specific Date of the Good Care to align with NHS requirements.
Submission cutover: Eligible completed events will now be submitted via the v6 API. Existing LFPSE reference numbers are retained.
Bug fixes and platform stability
This release resolves a wide array of defects to improve everyday reliability:
Forms and workflows: Resolved issues where conditional fields, text boxes, or submit and complete controls would occasionally fail to display or process correctly.
Actions and assignment: Saving Actions and assigning events to linked facilities now works consistently without error.
Reporting accuracy: Fixed facility selection lists in reporting configurations and stopped Insights Pro from occasionally duplicating event-count cards.
Communications: Linking notifications now accurately reference the correct facility, and time-based and misrecorded-draft emails are more reliable.
SOPs: Draft SOP files can now be correctly previewed, and sign-off assessments reliably reach all assigned users.
